18-Month Visit Planner is a free, online tool for parents to develop a personalized plan to effectively discuss their child’s developmental health with their health care team.
The Visit Planner covers five main topic areas: Physical Health, Safety, Parenting & Healthy Habits, Motor & Language Skills, and Social & Emotional Development. These topics are further broken down into sub-topics that parents can explore for more information. After completion, parents can save or print their personalized plan to bring to their 18-Month visit.
